Patient Care Coordination: Manage and coordinate patient care plans remotely, ensuring patients receive appropriate care and follow-up.
Telephonic Support: Provide telephonic assessments, patient education, and support to patients and their families.
Case Management: Collaborate with healthcare providers and multidisciplinary teams to develop, implement, and monitor individualized care plans.
Health Monitoring: Track and document patient health status, progress, and treatment outcomes using electronic health records (EHR).
Compliance: Ensure compliance with all regulatory requirements and maintain up-to-date patient documentation.

Qualifications

Active RN License: Current and unrestricted Registered Nurse license in the state of practice.
Experience: Minimum of 2 years of clinical nursing experience, preferably in case management or telehealth.
Technical Skills: Proficient in using EHR systems and telecommunication tools.
Communication Skills: Excellent verbal and written communication skills with a patient-centric approach.
Certifications: Case Management Certification (CCM) is a plus.
