We are seeking a compassionate and dedicated individual to join our team in providing comprehensive healthcare management services. In this role, you will play a crucial part in facilitating appropriate healthcare treatment, managing healthcare costs, and enhancing operational efficiency within our healthcare programs. Your focus will revolve around addressing clinical issues and ensuring the... well-being of our members.Responsibilities
Engagement and Support: Traveling throughout north Chicago and surrounding areas, you will meet face-to-face with our members, providing them with the care coordination, support, and education they need to navigate their healthcare journey effectively.
Care Management: Utilizing a variety of tools and resources, you will work to ensure that our waiver/LTSS members receive the appropriate healthcare outcomes. This involves assessing medical necessity, facilitating optimal outcomes, and identifying opportunities for continuous quality improvement and compliance.
Patient Advocacy: Empowering our customers is key. You will educate and support them, promoting compliance, satisfaction, and patient advocacy every step of the way.
Cost Optimization: Implementing and evaluating policies based on program directives, you will strive to optimize total costs while maintaining high-quality care standards.
Collaboration: Education and empowerment are essential. You'll work closely with colleagues at all levels, sharing knowledge and enabling informed decision-making.
Experience
Minimum of 2 years of experience in either behavioral health, social services, or human services field.
2+ years of experience in case management.
Willingness and ability to travel 50-75% of the time using your own vehicle, primarily in Peoria, IL, and surrounding areas. (Valid driver's license and reliable transportation required).
Education
Bachelor's degree in social services, human services, or behavioral health.
