Position Details:
• Part-time at 3+ hours per day
• Immediate start

Key Responsibilities: • Assessing psychosocial needs and provides services that enhance patient functioning, emotional well-being and continued access to services and resources in the community. • Providing care coordination • Providing psychoeducation • Providing referrals

Requirements: • Master’s degree in Social Work • Active Social Work License • Pennsylvania Teaching Certification • Virtual experience preferred
