JOB SUMMARY:
- The On-call Triage Supervisor is responsible for the operational oversight and management of all patient care activities and outcomes during their assigned shifts. The On-call Triage Supervisor ensures a culture of compliance, accountability, quality patient outcomes, and clinical excellence is maintained while adhering to the standards of quality care and contributing to the organization’s mission, vision, and values.
- Schedule: Part Time, Saturday/Sunday 8:30am-8:30pm
- MAJOR JOB FUNCTIONS:

- Supervises and coordinates the assignments of the on-call Registered Nurses and Licensed Practical Nurses of their assigned shifts; collaborating with the Director of Clinical Operations, or designee, if staffing requires adjustments to meet on-call demands.
- Answers all phone calls during assigned shifts, provides telephonic triage to patients, family, and caregivers, assigns nursing visits, as needed, and documents all communication and nursing assignments in the electronic medical record (EMR).
- Responsible for the review and completion of all pre-and post-admission workflow, approval, and submission of physician orders, and coordinating and scheduling on-call nursing visits.
- Responsible for overseeing all aspects of the referral to admission process by managing the patient intake process for referrals received during assigned shifts including initial contact with the patient/family/caregiver, obtaining signed consents, requesting medical records, attaching/uploading medical records into the EMR, verifying patient benefits/insurance, determining eligibility, collaborating with hospice physician/primary care physician/referral source, and coordinating/scheduling/assigning the admission visit.
- Assists on-call Registered Nurses and Licensed Practical Nurses in assessing, planning, implementing, and evaluating the patient and family/caregiver.
- Assists on-call Registered Nurses and Licensed Practical Nurses on assigned shifts with nursing visits as needed.
- Provides clinical guidance to all on-call clinicians to continually improve all aspects of care and ensures the quality and safe delivery of hospice services.
- Responsible for the maintenance of adequate and appropriate inventory supplies and equipment for the provision of patient care during on-call hours.
- Plans and implements the orientation program and continuing education program to meet the education and training needs of on-call Registered Nurses and Licensed Practical Nurses.
- Collaborates with the Director of Clinical Operations, or designee, to develop skills and techniques of on-call clinicians and in completing the evaluation of performance via performance improvement plans, productivity, and annual reviews of assigned on-call Registered Nurses and Licensed Practical Nurses.
- Fosters customer service and the building of relationships with referral sources and promotes hospice referrals in the healthcare community.
- Ensures the proper maintenance of clinical records and compliance with local, state, and federal laws.
- Maintains compliance with Local Medicare Review Policies (LMRPs) and Accreditation Commission for Health Care (ACHC) standards.
- Stays informed about changes in the field of nursing and hospice care, and shares information with appropriate organization personnel.
- Assume other duties as assigned by the Director of Clinical Operations or designee.
- REQUIRED EDUCATION/EXPERIENCE:
- Registered Nurse with current license to practice professional nursing in Colorado.
- Must live in Colorado
- Associate or bachelor’s degree in nursing from an accredited program preferred.
- Three (3) years of management or supervisory experience in hospice or related healthcare organizations preferred.
- Must have reliable transportation, proof of current automobile insurance, and a valid driver’s license.
