Job Summary

We are seeking a Full- Time, 40-hour LPN to support our practices onsite Monday through Friday from 8:30am to 5:00pm. We believe in providing exceptional care and that starts with exceptional people. We’re seeking an LPN to join our collaborative, patient-centered team!

Dedicated to clinical support, our nurses provide professional nursing care while collaborating with providers, staff, patients and families. This role is a mix of both patient facing and non-patient facing responsibilities. This is a unique opportunity to work autonomously as well as collaboratively with our team of 5 medical assistants.

    Responsibilities:
  • Responds to and prioritize incoming telephone calls, and triage appropriately.
  • Communicates with patients and family regarding medications, test results, and treatment instructions.
  • Vaccine/Medication administration and Management -Vaccine inventory
  • Population Health Management/Health Maintenance- Chronic Disease mgmt.
  • Observes, records and reports patient’s condition and plan of care to the physicians/provider.
  • Manages all aspects of process Prior Authorizations for: Rxs, DMEs, procedures, labs.
  • In-Basket management.
  • Ownership of Nurse Resource schedule for vaccines, BP checks, etc

Education
Associate's Degree Nursing required

Licenses and Credentials
Licensed Practical Nurse [LPN - State License] - Generic - HR Only required Basic Life Support [BLS Certification] - Data Conversion - Various Issuers preferred

Experience
medical experience 0-1 year preferred

    Knowledge, Skills and Abilities
  • Basic knowledge of patient education: interviewing and listening skills, teaching and engagement techniques, understanding of health-related conditions and information.
  • Ability to accurately perform vital signs and obtain pertinent health information including but not limited to; temperature, pulse, respirations, blood pressure, height, and weight.
  • Ability to administer medication; routes of administration may include injections, intravenous and oral medications.
  • Knowledge and ability to perform specific procedures and tests as appropriate to the clinical setting.
  • Ability to communicate in a professional, courteous, clear and concise manner.
