- Skills
- Bilingual (English/Spanish) highly preferred.
- Excellent verbal and written communication skills.
- Proficiency with required medical instruments and technology.
- Knowledge of computer-based data management programs.
- Sound understanding of federal and state regulations, including HIPAA and OSHA.
- Responsibilities
- Conduct comprehensive assessments, including annual wellness exams.
- Counsel and educate patients and families about available benefits.
- Document patient encounters thoroughly.
- Coordinate care with primary care providers and specialists.
- Maintain high standards of care as defined by the practice.
- Prescribe medications and order tests.
- Assist patients with accessing educational resources.
- Participate in integrated care team meetings.
- Qualifications
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
- Board certification for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
- Current and unrestricted DEA certificate.
- Two or more years of direct patient care experience.
- Education Requirements
- Relevant degree in Nursing or Physician Assistant studies.
- Education Requirements Credential Category
- Master’s degree preferred.
- Experience Requirements
- Experience in managed care/IPA/health plans.
- Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home.
