Pay Range : $37.50 - $43.50 per hour

Position Description :

Funda Mental Health seeks a mission-driven, BBS-registered Program Director to lead the development of our new Enhanced Care Management (ECM) program. This is a founding leadership role in an emerging program. The Director will provide both strategic oversight and direct care during the early phases, including acting as Lead Care Manager while caseloads grow. This is a part-time (20-hour / week) position initially, with the expectation of growing into a full-time leadership role as the program expands.

This individual will be responsible for overseeing day-to-day operations, ensuring fidelity to the CalAIM ECM model, supervising multidisciplinary staff, and partnering with Medi-Cal managed care plans to ensure effective service delivery to high-need members. This is a unique opportunity to support underserved communities through field-based, trauma-informed care coordination that emphasizes equity, collaboration, and whole-person support.

    Essential Job Duties and Responsibilities :
  • Deliver ECM services directly to a small caseload of members during the startup phase, including assessments, care plans, and outreach.
  • Develop initial ECM workflows, tools, and protocols in alignment with DHCS and MCP requirements.
  • Establish foundational infrastructure for ECM operations and lead program growth strategy.
  • Provide leadership and oversight of ECM program operations, ensuring compliance with all DHCS, MCP, and contractual requirements.
  • Supervise Lead Care Managers, Care Navigators, and other ECM staff, providing coaching, support, and performance feedback.
  • Monitor caseloads, staffing ratios, and care coordination quality to ensure high-touch, person-centered engagement.
  • Lead recruitment, onboarding, and training efforts for new ECM staff in alignment with program protocols and cultural responsiveness.
  • Maintain positive working relationships with Managed Care Plans and community partners.
  • Use data to drive program improvements, including encounter tracking, outreach engagement, and care plan completion.
  • Facilitate weekly team meetings, case conferences, and multidisciplinary collaboration.
  • Support field-based engagement strategies, safety protocols, and crisis response procedures.
  • Serve as the point of contact for ECM-related audits, reviews, and quality improvement efforts.
  • Collaborate with the Chief Clinical Officer to ensure integration of clinical supervision and care standards across ECM teams.
    Required Qualifications :
  • Master's degree in Social Work, Counseling, Marriage & Family Therapy, or related field.
  • Minimum active registration with the California BBS as an ASW, AMFT, or APCC
  • 3+ years of experience working with Medi-Cal populations, including individuals with SMI, SUD, homelessness, or justice involvement.
  • 2+ years of supervisory experience in behavioral health, care coordination, or social service settings.
  • Comfort navigating ambiguity and building new programs from the ground up.
  • Proven ability to lead interdisciplinary teams and ensure high-quality service delivery.
  • Strong understanding of trauma-informed care, motivational interviewing, harm reduction, and culturally responsive practices.
  • Reliable transportation and willingness to travel in the field; valid driver's license and insurance required.
  • Strong communication, documentation, and organizational skills; proficiency with EHR and Google Workspace tools.
    Preferred :
  • Licensed clinician with the Board of Behavioral Sciences of CA (LCSW, LMFT, LPCC)
  • Bilingual in Spanish or other threshold language.
  • Prior experience with ECM, Whole Person Care, or CalAIM programs.
  • Experience working within remote or hybrid team structures.

Work Environment and Physical Requirements :

This is a remote-first position based in San Diego County, with significant community-based fieldwork required. The ECM Program Director will maintain a home office as their primary administrative workspace, using personally owned equipment (e.g., laptop, smartphone). Reliable high-speed internet, a private and professional environment for phone / video communication, and access to secure cloud-based tools are essential.

Frequent travel throughout San Diego County is expected for home visits, community outreach, partner meetings, and mobile care coordination. Candidates must have reliable transportation, a valid driver's license, and active auto insurance. This role involves both sedentary desk work and physically active field engagement, including walking, driving, and navigating diverse environments.
