Role Description
- The Health Promotion Coordinator is responsible for tasks related to increasing compliance with performance measures and promoting comprehensive health care for members from birth to age 21.
- Enter information from the tracking forms into the Medical Management System
- Process Referrals
- Forward specific referrals to appropriate departments
- Maintain database and documentation
- Enter and update data in the Immunization database
- Outreach made to the parent/guardian of members whose immunizations are not up-to-date
- Contact members via telephone and/or mail on child health and wellness related topics
- Communicate with providers on member related issues
- Follow up calls are made to members who do not show to their scheduled doctor visit to identify possible obstacles to attending scheduled visits
- Outreach to members referred to services to verify that needs have been met
- Perform all other duties as assigned
- Qualifications
- 1 year of experience in health insurance, or other health care related field
- High-School Diploma or GED in general field of study
- Requirements
- Knowledge of Medicaid regulations and guidelines
- Knowledge of childhood immunization requirements
- Familiarity with medical terminology
- Computer experience in Microsoft Excel, Word, Outlook
- Effective time management skills
- Problem solving abilities
- Ability to prioritize work tasks to adhere to deadlines and identified time frames
- Typing 35-40 wpm
- Effective interpersonal and communication skills
- Work cooperatively, positively, and collaboratively in an interdisciplinary team
- Work respectfully and positively with members
